To be able to use information, first you need to be able to access it.

“One AirAsia Black Box Is Recovered,” Wall Street Journal, January 12, 2015 A8. The flight data recorder was recovered from the crash site of Flight 8501 off Indonesia.  The search continue for the cockpit voice recorder.

Think for a moment as to how you will figure out what happened when something goes wrong with your business and, tragically, people die. What information is available?  What information do you wish was available?  How do you make sure that information is available the next time?

One knowledge management model is Learn Before>Learn During>Learn After.  How do you get the information to Learn After an event if you don’t collect it During the event?
